YIWH 64th Anniversary Dinner

Your contribution in excess of $125 per person is tax deductible to the extent allowed by law.
Please remember to include your Corporate Gift Matching form.
If you would like to assist members with financial difficulty in attending the dinner, please enter your additional contribution in the space provided below.

Payment Information:

Member payments for dinner reservations must be received within 14 days of reservation.
Non-Member payments are due along with reservation.
To assist the Young Israel in safeguarding its financial health, payments not received in a timely manner will result in cancellation of dinner reservations.
To make payments by check, please print out the attached form and follow the instructions.
